In this role you will oversee the bespoke care and support of clients, helping them to achieve their full potential and experience an increased quality of life. You will support the Senior Clinical Team in the assessment process and setting up new care packages, as well as carrying out patient centered care plans. You will responsible for implementing precautions to safeguard the welfare and safety of staff, clients, by implementing all policies related to health, safety, security and risk whenever applicable. We work with our service users to tailor care to support their exact requirements, no matter how unique their circumstances are. We provide holistic support from professionals our clients can trust, whether it is temporary post-operative support, or a loved one requiring long-term palliative care in their own home, our team works with clients to create a care service tailored to their needs. Every day you will... undertake initial holistic nursing assessments in partnership with the service user, their family and healthcare workers, as well as developing individualised care plans to ensure goals are patient focused, realistic and achievable be responsible for the production of standard and bespoke risk management plans for clients, ensuring any identified high risk is escalated and acted upon, working with other stakeholders to plan, monitor and review effectiveness of any plan in place manage the delivery of client specific clinical training for staff within packages and delegate tasks ensure all clients allocated are regularly reviewed in accordance with company policies, contributing towards the implementation of CQC regulations, auditing documentation and escalating concerns What's in it for you?
