Triage Mental Health and Wellbeing Coach

The role of the Triage Mental Health and Wellbeing Coach is to work as part of Humber Teaching NHS Foundation Trusts YOURhealth service in Bridlington with the aim of improving the mental health and wellbeing of patients within the Bridlington Primary Care Network.

The role will support both the Primary Care Network and patients within to access low to moderate mental health support and is split into two unique roles. Firstly you will be required to provide triage support to patients presenting with low mood, worry or have concerns impacting their wellbeing. Triage clinics will be allocated to enable Primary Care to book directly into triage clinics for 20 minute contact calls to discuss the needs of the patient and to refer or signpost them onto the relevant service without the need of a GP consultation. The second element of the role is to provide longer term support to those with low to moderate level needs to help them move from surviving to thriving. The aim being to help people feel in control of their health and wellbeing by addressing mental health, general health, wellbeing and inclusion needs using a behaviour change approach and active recovery methodology including the Recovery Star with continuous goal setting and action planning throughout their time with the service.

The role of the Mental Health and Wellbeing Coach is a role integrated as part of the YOURhealth team whilst working clinically with the Primary Care Mental Health Network.
