Role Essentials :

  • Proven experience and passion for supporting older individuals
  • Understanding of older people's needs, including those with dementia
  • Excellent communication skills across various stakeholders
  • Ability to guide and signpost individuals to local and national support services
  • Talent for motivating and encouraging community connections
  • Team player with solo prowess
  • Proficient in IT, literacy, and numeracy
  • Guardian of confidentiality with adherence to policies
  • Satisfactory DBS check

Desirable Skills :

  • In-depth knowledge of local resources for older individuals
  • Experience in the care or voluntary sector
  • Understanding of safeguarding and statutory services provision
  • Ability to assess individual needs and risks
  • Flexibility for occasional evening and weekend shifts

Your Mission : As a Community Connector, you'll be the go-to person for older individuals seeking support and activities in their local community. Your role involves providing information, advice, and assistance to enhance independence and foster community connections.

Your Responsibilities :

  • Develop and maintain a database of local groups and support services
  • Stay updated on statutory and voluntary organizations
  • Offer friendly support and signposting to relevant services
  • Build partnerships to identify suitable support for individuals
  • Empower individuals to make informed choices
  • Promote Guild Care services and facilitate referrals
  • Monitor, evaluate, and report on performance
  • Champion feedback for continuous improvement
  • Uphold Guild Care policies and procedures
