As a registered health care professional and mental health practitioner you will work in a dual role with nursing colleagues and maintain your own professional identity. The post holder will provide strong clinical leadership, ensuring evidence-based practice is delivered to shape and improve the quality of services. The post holder will be responsible for establishing operational and clinical service mechanisms that supportsafe high-quality care throughout the service care delivery (8-6 pm, 7 days per week). As a member of the Integrated Mental Health Team (IMHT), you will provide flexible, responsive, proactive assessment, care, and management to individuals experiencing early signs of relapse, new acute phase mental illness, distress, or posing a risk in termsof self-harm. All interventions will be short-term; solution-focused and will build on maintaining the safety, wellbeing, and empowerment of the service user. The IMHT implements a Single Point of Access (SPA) and an Integrated Mental Health Team structure which providesindividualised care as defined by the stepped care model and crisis resolution provision. The PPG Wellbeing Approach is central to the care delivery. The Post-holder will work collaboratively with the prison, primary care, and other specialist clinicians to meet the needs of your patients to facilitate patient-focused, innovative, evidence-basedpractice, in line with NICE guidance and Practice Plus Group objectives.
