Clinical Health Support Worker

My job makes better lives by promoting health and independence for people with learning disabilities by giving practical support and following a plan of care developed by a team professional feeding back to the professional on the implementation and evaluation of the work that they are undertaking. Working within a Positive Behaviour Support framework to provide accessible, non-stigmatising, localized service that contributes to the specialist health care of people with learning disabilities within the locality.
