My job makes better lives by promoting health and independence for people with learning disabilities by giving practical support and following a plan of care developed by a team professional feeding back to the professional on the implementation and evaluation of the work that they are undertaking. Working within a Positive Behaviour Support framework to provide accessible, non-stigmatising, localized service that contributes to the specialist health care of people with learning disabilities within the locality. To participate in assessments, planning, implementation and evaluation of interventions as part of a holistic approach working in a variety of To observe and document any changes in wellbeing of the people that they are supporting, reporting to the professional or team manager any new or change in risk or presentation of the person. To take appropriate and immediate action to keep people safe in a crisis or safeguarding situation, reporting back to a senior colleague in a timely manner.
