Health Care Support Worker

An opportunity has arisen in the Community Mental Health Team (CMHT), Ty Einon Centre for a Health Care Support Worker (Band 3) to join our busy team. The CMHT operates Monday - Friday 9am - 5pm 5 days a week except Bank Holidays. We are looking for a motivated individual who can work independently and as part of a team. Good communication skills are essential as well as having a positive attitude to mental health care whilst ensuring individual needs for privacy, dignity, respect and confidentiality are upheld at all times. Working as a member of the CMHT will involve working within the guidelines of the Mental Health (Wales) Measure 2010 and keeping the Recovery ethos at the forefront of nursing care. The ability to speak Welsh is desirable but not essential for this role. To maintain a caseload. To support Care Co-ordinators in the day to day delivery of quality health care to patients predominately in the community. Reporting observations to the Care Co-ordinator. To assist in the delivery of medication management at designated clinics. To promote independence and self-care by supporting the Care Co-ordinator in the delivery of rehabilitation programmes. To reduce isolation through encouraging patients to take part in activities as identified in the Care and Treatment Plan. To communicate effectively and sensitively with patients, carers and family members. To maintain up to date knowledge of the different roles of the staff, from both the statutory and voluntary organisations. To adopt a professional approach, in liaising with other disciplines and agencies. To participate in CMHT meetings. Through regular appraisal/performance review, maintain and promote own development. To adhere to all relevant Health Board Policies and Procedures. Actively participate in the setting, monitoring and maintenance of standards of care within the CMHT. The post holder will be required to maintain accurate mileage records, timesheets, and expense claims in accordance with Health Board Financial Procedures.
